Launch of the Transis Lab pilot projects

The Transis Lab project is launching six pilot projects, with the aim of improving the quality of life of older adults living at home who are physically and/or socially fragile.

These pilot experiences of the Transis Lab project are organized around a quasi-experimental approach in rural and peri-urban areas in the Basque Country, Navarre, the Hautes-Pyrénées, and the Pyrénées-Atlantiques. Among the most innovative aspects are:

  • Personalized Activity Plans, co-created with seniors to promote their autonomy, social participation, and connection to local resources.
  • Digital assessment of fragility, before and after the intervention, thanks to a tool specific to the project.
  • Activation of community ecosystems, articulating support networks between social, health, and third-sector actors.
  • Qualitative methodologies that allow learning to be systematized and transferred to other territories.

These experiences not only seek to intervene, but also to transform local care systems, in close collaboration with rural communities.
